For organizations

The CAPA was closed.

The problem wasn't.

Something keeps recurring.

Inga Zollinger writing at her desk

A deviation resurfaces despite corrective action.

A finding returns under a different name.

Another CAPA closes — yet somehow the same underlying problem appears again.

You’ve reviewed the surface.
The surface isn’t where the answer lives.

When the same underlying pattern returns through different deviations, processes or audit findings, another corrective action may not be enough.

It may be time to look at the system differently.

I look for the pattern behind the problem.

For over two decades, I’ve led QA operations and compliance in pharma and medtech — currently as Head of QA Operations & Compliance at Ophtapharm, a Sentiss company.

That work taught me to read complex systems the way others read case files — and to distrust any fix that simply resets the clock until the next occurrence.

Before compliance, I trained as a linguist and worked for years as a legal interpreter. It taught me early that meaning is rarely contained in one statement, one document or one perspective.

That same instinct for finding what sits beneath a surface-level explanation is what I bring to chronic quality issues: the ones that keep resurfacing no matter how many CAPAs have already been closed.

Recurrence is a signal.
The question is whether the system is listening to it.

How the work moves.

  1. 01Diagnostic Call

A focused conversation to understand the pattern you’re seeing — not just the latest deviation, but what’s repeated before it and what hasn’t held.

  2. 02Root-Cause Investigation

A structured review of the quality system:

    • process
    • documentation
    • evidence
    • ownership
    • interfaces
    • decision-making
    • previous investigations
    • CAPAs
    • organizational patterns beneath the technical ones

I look at what the previous investigations may not yet have connected.

This is an additional outside perspective — one able to connect evidence across organizational boundaries that internal reviews are rarely positioned to cross.

  3. 03System Realignment

The investigation is translated into a practical plan to close the underlying gap: sequenced, prioritized, evidence-based, pragmatic and specific to the organization.

    Not another generic quality framework.

  4. 04Embedded Support · optional

For organizations that want the response implemented and stress-tested rather than simply diagnosed.

Ongoing advisory can continue through remediation and the next relevant audit cycle.

Engagement models.

Diagnostic Review

Single engagement

A focused investigation into one specific recurring issue.

Delivered as a clear findings report identifying likely systemic root causes and recommended corrective direction.

System Realignment

3–6 months

For patterns spanning multiple processes or repeat findings across audit cycles.

Combines investigation with implementation support.

Ongoing Advisory

Retainer · 6–12+ months

An independent root-cause perspective available on an ongoing basis — reviewing emerging deviations, supporting audit preparation and remediation, and identifying signs of pattern recurrence before they become chronic.

Scope and investment are confirmed following your Diagnostic Call, matched to system complexity.
